Your records are private and linked to your email. As administrator I have technical access to the database but will not look at your records — I will only ask for your feedback at regular intervals.

Honest reactions are the most valuable feedback you can give. If you prefer not to record real decisions during this phase that is completely fine — realistic but fictional examples are just as useful for testing the format. What I am testing is whether the forms feel natural, whether the fields make sense, and whether recording a decision takes the right amount of time and effort.
